Chiropractic Case History/Patient Information FORM
Dr Allen Culpepper began his experience with Chiropractic at the young age of 4 years old. His mother had previously had a lung fungal infection that the medical doctors had given up on, even though she was attended at the prestigious Emory University Hospital, and only upon the urging of family friend did they finally, as a last resort, take her to see the local Chiropractor. Needless to say she is still alive and well after 60 plus years of this incident. Then his sister came down with rheumatic heart disease that was also finally helped by Chiropractic and finally his father had a heart attack and was finally helped by the Chiropractor. Dr. Culpepper himself had extreme allergic reactions, from pets to foods, and was taken for Chiropractic adjustments and has not had any problems since. After all these results Dr. Culpepper’s father finally said, God must be telling us something, and so he quit his job at the downtown Atlanta, GA Sears distribution center and left with his family to Davenport Iowa in 1957 to attend Palmer College of Chiropractic. Thus began the Culpepper’s family journey into the world of Chiropractic. Dr. Allen Culpepper after obtaining his pre-Chiropractic education at West Georgia College and Georgia State University attended Palmer College, as his father had, in 1973. Upon graduation he returned to Georgia and practiced with his father in Mableton near Austell. He has had practices in Marietta, North Carolina and Spain, and now practices in Temple Georgia. Just prior to opening his practice at Temple Chiropractic in 2007 he had gained an interest in the mind body relationship and attended his old college, which had become a University in Carrolton West Georgia, in the Master Program of the Psychology Department thus gaining insight into the complexities of the mind and relationships, as well as the mind/body connection. In addition to the usual complementary techniques utilized by most Chiropractors his interest in the mind body led him to the B.E.S.T. (Bio Energetic Synchronization Technique) which is a soft touch technique used in the mind/body healing field. It is a system of health care that is state of the art balancing of the body/mind/memory/soul energy fields. When these energy fields are out of balance, just as in the traditional physical body explanation of balance/imbalance symptoms develop and health thus deteriorates. Many factors affect this balance; emotional issues from the past and present, choices that we make or do not make. Its foundation are the Six Essentials: what we eat and drink, how we exercise and rest, what we breathe, and what we think, all these affect t our energy fields and thus health. Additionally Dr. Culpepper uses the Cox Disk Traction Technique that is one of the first and longest utilized procedures to help correct disc protrusions, a significant producer of lower back pain. This technique helps to relieve the pain of compressed or herniated disk problems in the spine. It is virtually painless and has been shown to be very effective. Your first visit to Temple Chiropractic will give you an opportunity to find out if chiropractic care can help you. You will fill out a case history form, which will be gone over with you by Dr. Culpepper. Dr. Culpepper will then perform a thorough examination of the area of complaint and make recommendations accordingly. Dr. Culpepper will go over the findings with you and explain what he feels is causing your symptoms. Further tests may need to be ordered out. A written report of findings will be given to you to take home, outlining your schedule of care. Dr. Culpepper will then begin your recommended schedule of care. Your first day’s visit will last about an hour. Future visits will be about 10-20 minutes from the time you walk in to the time you check out. Dr. Culpepper is very conscious about waiting time in his office and makes every effort for you to be seen at your appointment time. |
